Patient was referred for diagnostic procedure and treatment of tooth #37.   By reason of pulp inflammation RCT was done 3 years ago. Last year the patient suffered a dull pain and feeling of heaviness around that tooth. RCT looked well done but there was a PAL on distal root. There was overextension of gutta-percha beyond the apex and it looked strange   because of the divergence of gutta-percha points. It dropped me the hint about probable VRF. The suggestion was confirmed after the composite filling was removed.
