Patient suffering from pain and  swelling  sent to RCT of 36 (19) with a suspicion of  perforation in coronal third of one of the mesials. No periodontal pockets but tooth had a high mobility

Preoperative X-ray shows bone resorption in furca area
Careful examination under the high magnification with DOM detected no defect of dentin walls
Three orifices of mesial system were found but not reason for inflammation in furca. Calcium hydroxide were placed as a temporary filling.
The matter was brought to light only on the final X-rays and it was a lateral canal. Note two ledges in MB created by RD in a effrot to negotiate the canal.
